ETS Event Forms

To attend an ETS event, you must complete the forms below. Additional forms for specific events will be sent to students individually as some are invite only.

Medical/Insurance and Emergency Contact Information Form

Must be signed by parent indicating emergency numbers for the event. We encourage you to send a copy of your insurance card. This ensures prompt care should there be a need. All prescription medicine MUST be turned over at check in and a member of the ETS Staff will dispense as indicated on your form. Students need to let us know about inhalers or other emergency medicines, but will be allowed to keep those themselves so that they have immediate access when necessary. Our medicinal policy is non-negotiable.

Field Trip Permission Slip

This allows us to transport your child to the event and back. You are responsible for transportation to and from Minnesota State University, Mankato.

Photo Release Form

We have a lot of fun and want to be sure and capture these moments for our students and ask that all students have a photo release allowing them to be in our group photos.
